FJA Online Intent to Continue ('12-'13) Form
FJA Board of Directors approved an Activity Fee to be requested at time of Intent to Continue submission.
$100.00 for Kindergarten through Sixth grade and $150.00 for Middle School grades Seven & Eight.
This fee will be used to help offset expenses of upcoming Academic field trips/Character Education activities during the school year.
An Arizona Tax Credit receipt will be issued at time of remittance. This can be applied on your State Income Tax return for credit.
